Admissions

The admissions process varies by what living option and level of services you need.


If you are looking for skilled services and currently have a loved one in the hospital, it is best to have the hospital social worker send us a referral. 


If you want assisted living or long-term care for a loved one who is at home, it is best to contact admissions directly to see what next steps are needed.


If you are in another facility and wish to transfer to AC Village, work with your social worker to send us a referral.

It is helpful if you have the insurance information, power of attorney and living will paperwork available. You will need to complete a financial disclosure if you are looking at long-term or assisted living care.

For assisted living, prospective residents need to be seen by a physician for a medical history, physical and a functional assessment prior to acceptance. This must be done 90 days before admission.

We do not operate from a waiting list for long term care, skilled, or assisted living.

We are here to service everyone in our community. You do NOT need to be a member of or attend the Apostolic Christian Church.
